Accepted Insurance Providers

At CAM Medical Group, we are committed to making quality healthcare accessible and affordable.

Our primary care physician, Dr. Cesar Morales, and our team of providers are in-network with most major commercial insurance plans, including Medicare, and HMOs (health maintenance organizations). Please review the list below to see if your plan is accepted.

Health Maintenance Organizations (HMOs)

We accept HMO plans from multiple networks, each with designated site numbers:

HMO plans require patients to select a Primary Care Provider (PCP) who coordinates all medical care and referrals. If you are enrolled in an HMO plan, please ensure Dr. Cesar Morales is listed as your PCP before scheduling an appointment. 

Out-of-Network Patients

If our providers are not in-network with your insurance, you can still receive care. However, full payment will be required at the time of service. We recommend checking with your insurance provider to determine if you’re eligible for reimbursement.

Billing & Payment

Payment is expected during service, and we offer flexible financing options to help make your care more accessible.

For your convenience, we accept most major credit cards, as well as cash and checks. Patients are responsible for deductibles, co-pays, and any out-of-pocket expenses determined by their insurance plan.

Billing & Insurance Guidelines

To ensure a smooth billing experience, please keep the following in mind:

  • Confirm your coverage – Check with your insurance provider to verify that CAM Medical Group is in-network with your plan and to clarify details about your coverage, including co-pays, deductibles, and any referral requirements. Remember: If you are enrolled in an HMO plan, please ensure Dr. Cesar Morales is listed as your PCP before scheduling an appointment. 
  • Bring your insurance card – Always bring your current one to each visit. If your insurance details change, please inform us promptly to avoid coverage delays.
  • Obtain prior authorization – Some treatments or procedures may require prior authorization from your insurance provider. If approval is needed, please obtain it in advance. Our team is happy to assist with the process, if necessary.
  • Respond to insurance requests – If your insurance company requests additional information, responding promptly can help prevent coverage delays and claim denials.
  • Understand your financial responsibility – Certain services may not be covered by your plan or deemed not medically necessary. In such cases, you will be responsible for the cost. If you have any questions, our billing team is here to help.
