Medicare Annual Wellness Exams

A Medicare Annual Wellness Exam is a yearly visit to help Medicare Part B beneficiaries stay proactive about their health. This exam is focused on reviewing your current well-being and potential health risks and developing a preventive care plan to support your long-term wellness.

During this visit, you will have an opportunity to discuss your lifestyle changes and health concerns and go over important preventive care steps. This exam is fully covered by Medicare Part B, meaning those eligible incur no out-of-pocket costs.

During your Medicare Annual Wellness Exam, our team will guide you through several key assessments, including:

• Health risk assessment – You complete a simple questionnaire to help our providers understand your current health status, lifestyle, and potential risks.
• Routine measurements: Our physicians check your height, weight, blood pressure, and other vital signs to monitor any changes in your health over time.
• Medical and family history review – We review your past medical history, chronic conditions, and family health history to identify any hereditary risks or trends in your well-being.
• Medication review – Our team assesses your current prescriptions, over-the-counter medications, and supplements to ensure they are appropriate for your health needs.
• Preventive screening schedule - We create a customized checklist of recommended screenings and vaccinations to help you stay on top of your preventive care needs.
• Cognitive health assessment - Our team will screen you for early signs of cognitive conditions like dementia or Alzheimer’s disease. They will look for indicators such as difficulty remembering, learning new things, or concentrating.

After your assessments, our physicians will combine your physical health data with your health risk assessment results to develop a long-term preventive care plan. 

This plan helps you stay proactive by outlining when to schedule vaccinations, complete preventive screenings, and book follow-up visits to support your overall well-being.

A Medicare Annual Wellness Visit focuses on preventive care and health planning. It does not include a full physical exam, vaccinations, lab work, or preventive screenings, such as blood tests or imaging.

During this exam, our only focus is on reviewing your overall health and developing a personalized preventive care plan. This appointment does not cover treatments, tests, or vaccines. Please schedule a separate appointment if you require these services.

The Medicare Annual Wellness Exam is fully covered for those enrolled in Medicare Part B. This means there is no cost, copay, or deductible for the visit itself as long as your provider accepts Medicare assignment. 

You qualify for a Medicare Annual Wellness Visit if:

• You have had Medicare Part B for at least 12 months.
• It has been at least 12 months since your last Welcome to Medicare Visit or your previous Annual Wellness Visit.

If you are within your first 12 months of Medicare Part B enrollment, you may qualify for a Welcome to Medicare Visit (Initial Preventive Physical Exam, IPPE) instead, which is also covered by Medicare.

Please note that if you get additional tests, screenings, or treatments during the same visit that are not covered under this preventive benefit, you may have to pay coinsurance or meet the Part B deductible.

Follow these guidelines to prepare for your Medicare Annual Wellness Exam:

• Review your health history – Be ready to discuss past illnesses, surgeries, and any family history of health conditions.
• Make a list of your medications – Note all prescriptions, over-the-counter medications, and supplements you take, and bring the list with you.
• Write down your health concerns – Take note of any recent changes in your health, ongoing concerns, or questions you would like to discuss with the provider.
